1. ACCEPTANCE OF TERMS

Welcome to FACE Secrets Florida (“FACE Secrets,” “we,” “us,” or “our”). By accessing our website at facesecretsflorida.com (the “Site”), scheduling appointments, receiving treatments, or using our services, you (“Client,” “you,” or “your”) agree to be bound by these Terms and Conditions (“Terms”).

If you do not agree to these Terms, please do not use our Site or services.

These Terms constitute a legally binding agreement between you and FACE Secrets Florida, a mobile medical spa operating in Florida. We reserve the right to update, change, or replace any part of these Terms by posting updates on our Site. Your continued use of the Site or services following the posting of changes constitutes acceptance of those changes.

2. SERVICES PROVIDED

FACE Secrets Florida is a mobile medical aesthetics practice providing the following services:

• Neurotoxin injections (BOTOX®, Dysport®, Xeomin®, Daxxify®)
• Dermal filler injections (hyaluronic acid-based)
• PDO thread lifts (smooth and barbed threads)
• Medical-grade skincare consultations and products
• Aesthetic consultation and treatment planning
• Other medical aesthetic services as described on our Site

Mobile Service Model:
All services are provided at a location of your choice (home, office, private venue, yacht, etc.) within our service area in South Florida. We do not operate a fixed clinic location.

Medical Director:
All treatments are performed under the supervision and direction of Dr. Daudrie Ann Powell, a licensed medical professional in the State of Florida.

3. ELIGIBILITY & AGE REQUIREMENTS

You must be 18 years of age or older to receive services from FACE Secrets. By booking an appointment, you represent and warrant that you are at least 18 years old and have the legal capacity to enter into this agreement.

Services are provided only to individuals who are:

• In general good health
• Not pregnant or nursing (for injectable treatments)
• Free from contraindications to the requested treatment
• Able to provide informed consent

Minors: We do not provide aesthetic services to individuals under 18 years of age, even with parental consent.

4. APPOINTMENT BOOKING & DEPOSITS

4.1 Booking Process

Appointments may be booked through:

• Our website booking system
• Phone consultation with Dr. Powell
• Email request
• Third-party scheduling platforms integrated with our services

4.2 Deposit Requirement

A $150 non-refundable deposit is required to secure all appointments. This deposit:

• Reserves Dr. Powell’s time and resources
• Will be applied to your treatment cost at the time of service
• Is forfeited if you cancel within 48 hours of your scheduled appointment or fail to appear (no-show)

4.3 Cancellation & Rescheduling Policy

• More than 48 hours notice: Full deposit refunded or applied to rescheduled appointment (your choice)
• Less than 48 hours notice: Deposit forfeited; you may reschedule with a new $150 deposit
• No-show (failure to appear): Deposit forfeited; future bookings may require payment in full upfront

Emergency Exceptions:
We understand emergencies happen. If you experience a medical emergency, family crisis, or other extenuating circumstance, please contact us immediately. We will work with you on a case-by-case basis.

4.4 Late Arrivals

If you are more than 15 minutes late to your scheduled appointment, we reserve the right to:

• Shorten your appointment to accommodate Dr. Powell’s schedule
• Reschedule your appointment (deposit forfeited)
• Cancel your appointment (deposit forfeited)

Please notify us immediately if you are running late. We will do our best to accommodate you.

5. PRICING & PAYMENT

5.1 Pricing

• Pricing is customized based on individual needs and determined during consultation
• We do not post pricing on our website due to the personalized nature of treatments
• All pricing will be clearly communicated before treatment proceeds
• Prices are subject to change without notice

5.2 Payment Methods

We accept:

• Credit cards (Visa, Mastercard, American Express, Discover)
• Debit cards
• Cash
• Cherry financing (subject to Cherry’s approval and terms)

5.3 Payment Terms

• Payment is due at the time of service
• Your $150 deposit will be applied to the total treatment cost
• For treatments financed through Cherry, you agree to Cherry’s separate terms and conditions
• Outstanding balances must be paid before future appointments can be scheduled

5.4 Petal Program Redemptions

• Petal Program points may be redeemed for services and products as outlined in the Petal Program Terms
• Petal redemptions may be combined with cash/card payments
• Petal redemptions cannot be combined with certain promotional offers (restrictions will be clearly disclosed)

6. INFORMED CONSENT & MEDICAL HISTORY

6.1 Medical Consultation Required

All clients must complete a medical history questionnaire and participate in a consultation with Dr. Powell before receiving treatment. You agree to:

• Provide accurate and complete medical information
• Disclose all medications, supplements, and medical conditions
• Inform us of any allergies or prior adverse reactions to treatments
• Update us on any changes to your medical status

6.2 Informed Consent

Before any treatment, you will receive detailed information about:

• The nature and purpose of the treatment
• Expected results and potential variations
• Possible risks, side effects, and complications
• Alternative treatment options
• Post-treatment care requirements

You acknowledge that:

• You have the right to ask questions and have them answered
• You understand the treatment and its risks
• You consent to the treatment voluntarily
• No guarantees have been made regarding specific results

6.3 Refusal of Treatment

Dr. Powell reserves the right to refuse or discontinue treatment if:

• You have contraindications that make treatment unsafe
• You fail to disclose relevant medical information
• You are under the influence of alcohol or drugs
• You exhibit threatening, abusive, or inappropriate behavior
• Treatment would not be in your best medical interest

7. TREATMENT RISKS & DISCLAIMERS

7.1 Understanding Risks

All medical aesthetic treatments carry inherent risks. Common risks include but are not limited to:

• Bruising, swelling, redness at injection sites
• Allergic reactions
• Infection (rare when proper protocols are followed)
• Asymmetry or unsatisfactory aesthetic results
• Migration of product
• Nerve injury (rare)
• Other complications specific to each treatment type

You acknowledge that:

• Dr. Powell has discussed these risks with you
• You accept these risks
• Results vary by individual
• Multiple treatments may be needed to achieve desired results

7.2 No Guarantees

FACE Secrets does not guarantee specific results. Aesthetic treatments are highly individualized, and outcomes depend on factors including but not limited to:

• Your unique anatomy and physiology
• Skin type and condition
• Age and lifestyle factors
• Adherence to pre- and post-treatment instructions
• Individual healing response

7.3 Limitation of Liability

To the fullest extent permitted by law, FACE Secrets, Dr. Powell, and our affiliates shall not be liable for:

• Indirect, incidental, consequential, or punitive damages
• Lost profits, lost revenue, or lost data
• Damages arising from your failure to follow pre- or post-treatment instructions
• Results that do not meet your subjective expectations (when treatment was performed according to standard of care)

Our total liability for any claim related to services shall not exceed the amount you paid for the specific treatment giving rise to the claim.

7.4 Medical Emergencies

In the unlikely event of a medical emergency during or after treatment:

• Call 911 immediately
• Notify Dr. Powell as soon as possible
• Seek emergency medical attention

Dr. Powell will provide emergency contact information and post-treatment support, but you acknowledge that immediate emergency care may require hospital-based intervention.

8. POST-TREATMENT CARE & RESPONSIBILITIES

8.1 Your Responsibilities

After treatment, you agree to:

• Follow all post-treatment care instructions provided by Dr. Powell
• Avoid activities that may compromise results (as specified for each treatment)
• Report any unusual symptoms, adverse reactions, or concerns immediately
• Attend any recommended follow-up appointments
• Avoid self-treating complications (contact Dr. Powell first)

8.2 Our Responsibilities

We will:

• Provide clear, written post-treatment care instructions
• Make Dr. Powell available for questions and concerns during recovery
• Address complications promptly and professionally
• Provide follow-up care as medically necessary (additional fees may apply)

8.3 Follow-Up Appointments

Some treatments require follow-up appointments for assessment, adjustment, or touch-ups. These may be:

• Complimentary (as specified during initial treatment)
• Subject to additional fees (clearly communicated in advance)
• Necessary to achieve optimal results

Failure to attend recommended follow-up appointments may affect your results. We are not responsible for suboptimal outcomes if you do not complete the recommended treatment plan.

9. MOBILE SERVICE TERMS

9.1 Service Area

We provide mobile services within our designated service area in South Florida. Service area boundaries are subject to change and will be confirmed when you book.

9.2 Location Requirements

You are responsible for providing:

• A clean, private, well-lit space for treatment
• A comfortable surface (bed, treatment table, reclining chair)
• Access to running water and a restroom (preferred but not always required)
• A safe environment free from hazards
• Privacy from interruptions during treatment

9.3 Environmental Conditions

We reserve the right to postpone or relocate treatment if:

• The location is unsafe or unsanitary
• Lighting is insufficient for safe treatment
• Privacy cannot be maintained
• Environmental conditions compromise sterility

In such cases, we will work with you to reschedule or find an alternative location. Deposits may be retained or refunded at our discretion based on the circumstances.

9.4 Travel & Mileage

• Travel within our standard service area is included in treatment pricing
• Travel outside our standard service area may incur additional mileage fees (disclosed in advance)
• Excessive travel time due to inaccurate address information may result in shortened appointment time

10. PHOTOGRAPHY & MARKETING

10.1 Before & After Photos

With your written consent, we may take before-and-after photos for:

• Your medical records
• Treatment planning and follow-up
• Marketing and promotional purposes (only with separate authorization)

You have the right to:

• Decline photography entirely
• Consent to photos for medical records only (not marketing)
• Revoke consent for marketing use at any time

10.2 Testimonials & Reviews

If you provide a testimonial or review:

• We may use it on our website, social media, or marketing materials
• We will not use your full name without permission
• You represent that your testimonial is truthful and based on your actual experience

10.3 Social Media

If you tag FACE Secrets or Dr. Powell in social media posts:

These Terms, together with our Privacy Policy and any treatment-specific consent forms, constitute the entire agreement between you and FACE Secrets regarding your use of our services and supersede all prior agreements or understandings.
